Accessibility

Both surgeries have disabled access and disabled toilets.    

All staff members are willing to provide assistance should it be required.  If you need assistance, please let reception know.

Registering with the practice

To register with either practice simply ask at reception for a registration form.  You will be asked to complete 2 forms a registration form and a new patient questionnaire.

Our practice area covers Penywaun, Trecynon, Llwydcoed, Gadlys, Aberdare, Cwmbach, Aberaman, Abercwmboi and Fernhill.

 

Student Training

The practice may host clinical students such as trainee Doctors.  They will be accompanied by one of our regular clinicians.  You will be asked to complete a consent form before the consultation.  You have the right to decline for them to be present for your consultation.

How to book an appointment

By telephoning the surgery from 8am Monday to Friday.

Calling into the surgery from 8.30am to book pre-bookable appointments.

 

Reception staff will ask you questions about why you need the appointment and will sometimes direct you to alternative services.

 

We offer a same day consultation for children 16 years of age and under with acute presentations.

 

We currently offer a variety of appointments:

Routine pre-bookable appointments

These can be booked up to 2 weeks in advance.

Online appointments

We offer online appointment via the NHSWalesApp

On the day telephone appointments.

These are available from 8am daily.  If the Doctor feels you need to be seen face to face they will arrange a time for you to attend the surgery.

Urgent appointments 

If there is no on the day appointments available and you feel that it can not wait until the next pre-bookable appointment or the next day our reception staff will take your details and will pass them on to be triaged by the Doctor.  

Home visits

To request a home visit please call the surgery before 10.00am if possible.  Home visits are at the Doctors discretion. The Doctor will telephone to triage the call and arrange a date or time to call. Home visits are only available for those patients whose medical and physical condition makes it impossible to attend the surgery.

Services available

The surgery offers the following services (appointments only):

Urgent and no-urgent GP appointments, cervical screening, baby clinic, post-natal checks, contraception, coil clinic, blood pressure monitoring, phlebotomy, immunisations, travel vaccinations, INR clinics, anticoagulation monitoring, wound care, health screening clinics for Asthma, Diabetes, COPD.

Repeat medication

Please allow 72 hours (3 working days) to process your repeat medication request (excludes weekends and bank holiday).

Please allow up to 5 days if requested to collect from a chemist.

You can request your repeat prescription by:

  • By Hand - Putting your paper repeat slip into the box on reception.
  • Pharmacy Collection - Via a nominated chemist 
  • By post  - (please enclose a self addressed envelope) 
  • Online - Via the NHS Wales APP

 

Unfortunately, we cannot accept requests over the telephone in the interests of patient safety.

 

Care homes

  • By Hand - Care home staff can hand repeat slips to reception staff.
  • Pharmacy Collection - Via a nominated chemist
  • E-mail - Acute and missing items can be requested via e-mail

 

Non NHS

If you require the Doctor to fill in a private form the request should be made via reception staff. If the request incurs a charge this is payable in advance of completion. The surgery will contact you to let you know the fee before completing the form. 

Test results

You can telephone or call into surgery for you test results.  All test results are given out after 2pm. 

 

Veterans

Have you ever serviced in the UK armed forces as a regular, reservist or thought national service?

Please let us know you may be eligible for priority treatment/referrals.

 

Patients rights

You have the right to be registered with a GP practice and receive care from them.

You have a right to ask to see a Doctor or Healthcare professional of your choice.

Where possible we will do our best to accommodate your request. Please let the reception staff know when booking your appointment.

In some cases it may not always be possible but we will always try to meet your request whenever we can. 

Patients are entitled to access to their medical records.  You will need to complete a form and provide Photo ID.  

You have a right to request a chaperone to be present during any consultation. We respect your dignity, privacy and religious beliefs.  If you require a chaperone please ask on reception or during the consultation.

 

Patients responsibilities

  • Attend the appointment on time
  • If it is no longer convenient or you no longer need the appointment please let the surgery know as early as possible.  You can also cancel appointment via the NHS APP.
  • Appointment are booked for the person who has made the appointment only.  We are unable to see more than one patient in an appointment.
  • Please treat the Doctors and staff with respect. 
  • To put repeat medication requests in on time.
  • To contact surgery for results

 

Zero tolerance

As a practice we operate a Zero Tolerance Policy.  We will not tolerate aggressive or abusive behaviour (verbal or physical) towards any of our staff or patient in the surgery.  Any person behaving in an unacceptable manner will be removed from the premises and may be removed from the practice list.

Any actual physical violence towards staff will ALWAYS be prosecuted.

 

DNA Policy

Due to a high incidence of patients not turning up for appointment we ask that if you book an appointment and no longer require it please cancel it

 

Complaints and concerns

If you have any complaints or concerns regarding your care or about the service we provide you can raise your concern with the Practice Manager, Ceri Moss.

Should you have a complaint your first port of call should be to contact the Practice manager at the surgery as described in the complaints procedure (please as at reception for a copy).

If you wish to raise a concern and are not the patient involved, we will require written consent from the patient to confirm that they are unhappy with their treatment and that we can deal with someone else about it.

Emergency information

Call 999 for urgent medical help if you are experiencing any of the following:

Drooping on one side of the face, slurred speech, difficulty raising your arms, weakness or numbness on one side of your body.
